What is a Vascular Occlusion?

A vascular occlusion happens when dermal filler enters an artery or presses on it from outside, cutting blood flow to the tissue it supplies. Without oxygen, that skin can start to die, so the first minutes matter. Occlusions are uncommon, but every filler injector will meet one eventually. Watch for: • Immediate blanching or white patches along a vessel path • Pain out of proportion to the procedure (lidocaine in the filler can hide it) • Mottled, dusky or lace pattern discolouration developing over minutes to hours • Slow capillary refill compared with the surrounding skin • Any change in vision, eye pain or droopy eyelid, which is an emergency needing immediate escalation

What is a Vascular Occlusion?

A vascular occlusion (VO) is when a blood vessel becomes blocked, reducing or stopping blood flow to nearby tissue. In aesthetic treatments, this can happen during dermal filler procedures if the filler is accidentally injected into a vessel or presses against it from the outside. Because blood delivers oxygen and nutrients, any blockage can deprive the tissue of what it needs to survive. Although vascular occlusions are uncommon, they are considered a serious complication and need to be addressed immediately to avoid lasting damage. Early signs, such as skin blanching, unusual pain, or changes in skin colour are critical warning signals. Prompt recognition and quick treatment make a significant difference in outcomes, which is why it's essential for practitioners to know what to look for and how to respond without delay.

Cosmetic Nurse Scripting in NSW: Ready for the 5 November 2026 Changes

From 5 November 2026, the Medicines, Poisons and Therapeutic Goods Act 2022 and its 2026 Regulation replace the NSW Poisons and Therapeutic Goods Act 1966. Cosmetic use substances, including botulinum toxins, hyaluronic acid fillers and deoxycholic acid, now carry their own rules.

What changes for NSW nurse injectors:

  • The doctor or NP must personally review the patient in person or by video before prescribing or writing a direction.
  • A verbal direction only works if the prescriber is physically in the room.
  • Nurses must record each administration, including the batch number, and send a copy to the prescriber and the business.
  • Clinics must keep emergency equipment on hand and ensure nurses are trained to respond to medical emergencies.
  • Penalties range up to 400 penalty units for individuals and 2,000 for corporations.

Clinics in other states are supported under their own state’s medicines laws, which differ. Queensland, for example, has two separate business models for nurse led clinics.
